  • DTI/SEC/CDA Registration
  • BIR Tax Returns PCY (stamped received)
    • Monthly Income Tax
    • Quarterly Income Tax
    • Annual Income Tax
  • Financial Statements PCY
    • Unaudited Balance Sheet and Income Statement as of December 31
    • Audited Balance Sheet and Income Statement (stamped received by BIR) to be submitted not later than april 20 of the current year
  • Contract of Lease (if rented)
  • Sanitary Permit

Note: Applicant may avail of early assessment at the Bureau of Fire Protection (BFP) and Office of the Building Offical (OBO).
